The team at Pacific Eye is proud to announce that we have a new digital referral process in place! You now have the capability to refer your patients to us in a streamlined digital format.

• No sign-on or login credentials are required
• The referral form is simple. We will ask for referring physician information, patient information and PES provider preferences
• Patient notes can also be added as required
• The referring physician information will remain stored and will not need to be re-entered in future submittals, making it easier and faster

Better Communication!

• Referring physicians will receive an email confirmation upon referral submission
• Additional notifications will be sent along the patient journey
• We will let you know when patient has been contacted by the Patient Access Team and when an appointment is scheduled
• Should the Patient Access team be unable to complete the scheduling process, a notification email will be sent explaining the reason why
